Unable to find "Community Toolbar for Socialize Template" Module

ISSUE SUMMARY:
I have uninstalled the socialize template and the Community Toolbar for Socialize Template module and then re installed it.
It has shown that the installation was successful, but i'm not able to find the toolbar module in the module manager.

STEPS TO REPLICATE:
1. Install Community Toolbar for Socialize Template module 'mod_community_toolbar_V2.1.1.zip'
2. After installation, go to Extensions>module manager
3. Unable to find the module here.

RESULT:
unable to find the module in module manager

EXPECTED RESULT:
Module must be shown

Hi,

please check it. it's joomla issue. you need to re-add it by clicking "new" at joomla menu manager.
